Rentive dispatch and alert program
Vendor and Maintenance Team SMS Enrollment Form and Dispatch Terms
This form (the “Form”) governs enrollment in the Rentive dispatch and alert program. The property management company named in Section 1 (the “PMC”) uses Rentive, a 24/7 AI-enabled maintenance-communications service operated by Rentive LLC, to receive and manage tenant maintenance requests and to send operational dispatch offers and ticket alerts to the PMC's selected vendors, technicians, and maintenance personnel. Rentive LLC is the registered SMS sender and a technology and communications provider that sends these messages by an automated system on the PMC's behalf.
Rentive is not a property manager, contractor, employer, agent, insurer, guarantor, or party to any recipient's work arrangement. A vendor or recipient is an independent contractor and is not an employee or agent of Rentive. Rentive does not supervise, direct, or control the manner in which any recipient performs work and is not responsible for a recipient's acts or omissions.
Only enrolled numbers, that is, numbers listed in Section 2 and consented to by signature in Section 4, are loaded into the dispatch and alert pool. A number that is not enrolled here is never texted. A person who does not consent to texts is not enrolled in this program; the PMC can still reach that person by phone or email outside it.

2. Dispatch and alert roster
- Dispatch receives job-dispatch offers and may reply YES or NO to accept or decline a job. Priority sets the order in which offers are made.
- Alert receives notice that a ticket was created and need not accept a job.
- After hours marks a dispatch recipient who may be offered jobs outside the PMC's business hours, including nights, weekends and holidays. Others are offered jobs during business hours only.
A person may be designated for both Dispatch and Alert. For third-party vendors, use the “Role / company” and “Trade” columns to record the vendor business and service category. Each person listed must check both boxes and sign the corresponding record in Section 4, and the mobile number in that record must match this roster exactly.

Dispatch cascade. If the priority-1 recipient does not accept within the configured window (default 15 minutes), the offer moves to the next dispatch recipient in priority order, and the PMC is alerted if nobody accepts. The PMC sets the frequency, timing, response periods, and priority order during onboarding. A recipient's YES, NO, or other response is subject to the PMC's configured dispatch rules and does not create a contract with Rentive.
Work is between the recipient and the PMC. Work, pricing, scope, access, insurance, licensing, safety, invoicing, and payment arrangements are exclusively between the recipient and the PMC or the applicable property owner. Neither Rentive nor the PMC guarantees work volume, priority, exclusivity, response time, available assignments, or payment.
3. What the messages are
Rentive sends the following operational SMS messages under this program on behalf of the PMC; it does not send marketing or promotional messages under this Form. Messages are sent by an automated system; a reply of YES or NO is read by that system and recorded on the ticket.
- Job-dispatch offers (to dispatch recipients): a short description of an available maintenance job, which may include property or unit information and access or scheduling information, asking the recipient to reply YES or NO followed by the job code shown in the message, to accept or decline. Status and follow-up messages may be sent about an accepted assignment.
- Ticket alerts (to alert recipients): a brief notice that a maintenance ticket was created, with the key details. Alert recipients need not accept a job unless they are also designated for Dispatch.
Message frequency depends on maintenance volume and on the recipient's place in the dispatch order. A recipient may receive multiple messages during an active issue and no messages when there is no relevant activity.
4. Individual SMS consent and signature
The following statement applies to every person listed in Section 2. Each enrolled person must check both boxes and sign the corresponding record below.
“I have read this Form and voluntarily consent to receive the operational SMS messages described above from Rentive, sent from the PMC's sending number in Section 1 on behalf of the PMC, at the mobile number listed next to my name in Section 2 and written below. I understand that message frequency varies with maintenance activity; I may receive multiple messages during an active issue and none when there is no relevant activity.
I represent that I am the subscriber, customary user, or authorized user of that mobile number and have authority to consent to receive these messages. I will promptly notify the PMC if the number changes, is reassigned, or should be removed from the roster.
Standard message and data rates may apply. Message delivery is not guaranteed and depends on carrier conditions. Consent is not a condition of any purchase, and not consenting only means I am not enrolled in this text program. I can reply STOP at any time to stop receiving messages, and HELP for help. This consent is limited to operational dispatch and alert messages; no marketing messages will be sent. Program terms: rentive.ai/terms#sms. Privacy: rentive.ai/privacy.”

Copy a consent record for each additional vendor, technician or recipient. A Section 2 number may not be enrolled unless the corresponding person checks both boxes and signs the Section 4 record.
5. Vendor participation and business information
These terms govern every vendor or contractor that uses the Rentive program to receive, schedule, perform, communicate about, or invoice a job, whether the vendor was designated by the PMC or later joined a Rentive vendor network. A vendor accepts these terms by checking the Section 4 box and signing this Form, by accepting work communicated through the program, or by continuing to use the program after notice of these terms.
To operate the program, a vendor may provide, or cause the PMC to provide, the vendor's business information: its business name, contact name, trade or service category, mobile number, dispatch preferences and, where the PMC records them, licensing and insurance details and service areas. The vendor authorizes Rentive to use that information, together with the vendor's response history in the program, to operate, administer, support, and improve the program, to maintain program records, and to create de-identified and aggregated data. Rentive may use and commercialize only the de-identified and aggregated data. These authorizations survive termination to the extent necessary for retained records and de-identified or aggregated data.
The vendor's pay, insurance, and work scope are matters between the vendor and the PMC or the applicable property owner, not Rentive. The PMC is responsible for the accuracy of the information it enters about the vendor.
6. Recipient duties; confidentiality; permitted use of information
Each recipient acknowledges and agrees that:
- Rentive does not select vendors; direct or supervise work; verify qualifications; confirm emergency conditions; or guarantee that program information is complete or accurate. Dispatch messages relay what the caller and the PMC reported.
- Before entering a property or commencing work, the recipient will independently confirm the scope, location, access authorization, safety conditions, and the PMC's authorization.
- Each vendor is responsible for maintaining all licenses, permits, insurance, training, and legal compliance required for its work.
- Personal and nonpublic information received through the program, including resident, owner, PMC-staff, tenant, unit, property, access, maintenance, and contact information, is not the recipient's information. The recipient may use it only to evaluate, schedule, perform, complete, communicate about, and invoice the related job, and to retain records required by applicable law.
- The recipient will keep that information confidential and secure; will retain it no longer than necessary for the permitted purposes or applicable-law recordkeeping; and will delete it on Rentive's or the PMC's written request unless applicable law requires retention.
- The recipient will not use that information to market or advertise its services; add it to a mailing, texting, or calling list; solicit work outside the program from a resident, owner, or property manager introduced through the program; or sell, rent, license, disclose, or otherwise make it available to another person. These restrictions continue after participation in the program ends.
- The recipient will not forward program messages, or any entry, lockbox, or access details they contain, to unauthorized persons.
- If the recipient learns that protected information has been used or disclosed contrary to this Form, the recipient will notify Rentive at support@rentive.ai and the PMC without unreasonable delay.
A breach of this Section is grounds for immediate suspension or removal of the recipient from the dispatch and alert roster, without limiting any other available rights or remedies.
7. Company authorization
The enrolling representative confirms that the people listed in Section 2 are part of the PMC's maintenance network or team; that each person has checked both boxes and signed the corresponding Section 4 record with a mobile number matching the roster; and that the PMC will retain this completed Form as its consent record.

Electronic signature: this Form and each enrolled-person record may be signed electronically. An electronic signature has the same legal effect as a handwritten signature, and each signer intends its electronic signature to bind that signer. A copy is available to each signer on request.
8. Opt-out, help, and recordkeeping
- A recipient can stop messages at any time by replying
STOP(also honored: STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT, REVOKE, OPTOUT) to any Rentive message; a STOP reply to the sending number is applied by the messaging system when it is received. A recipient can also opt out by any other reasonable means through the PMC; such requests are honored promptly and no later than 10 business days after the request. After opting out, the recipient receives one confirmation message and no further messages unless the recipient repliesSTARTto resubscribe. - A recipient can reply
HELPor INFO for assistance, or contact the PMC or support@rentive.ai. - Opting out removes the number from the active dispatch and alert pool. It does not affect an already accepted assignment or any separate agreement or work arrangement between the recipient and the PMC or the applicable property owner.
- Standard message and data rates from the recipient's wireless carrier may apply. Message delivery is not guaranteed and depends on carrier conditions.
- The PMC keeps this completed, signed Form as its consent record for TCPA and A2P 10DLC compliance for at least four years after the last message to any number on it. Rentive does not store this Form; it keeps each number's opt-out status and the message log.
9. Privacy and program information
The PMC and Rentive may process a recipient's name, phone number, response history, vendor business information, and assignment-related communications to operate the dispatch program, maintain records, support the service, and exercise the rights stated in this Form. Full SMS program terms: rentive.ai/terms#sms. Privacy Policy: rentive.ai/privacy.
This Form is solely an SMS enrollment and acknowledgment. It does not create an employment, agency, partnership, joint venture, exclusive-provider, or service-contract relationship between a recipient and Rentive. It does not obligate the PMC to offer work or any recipient to accept it.
